Under Dr. Granite's Immunization Business, we are offering flu and pnemonia vaccines again this year.   2010-11 Flu Season Vaccines will be available starting Wednesday, September 8, 2010 to our patients and the public.  We do not bill insurance companies with the exception of Medicare, which we must bill by law.  We accept cash, check (with valid driver's license), debit and credit cards.  We will also have clinic dates as follows:

MEMORANDUM

 

               TO:  All Existing Patients with United HealthCare and subsidiaries coverage

         FROM:  David S. Granite, M.D., F.A.A.F.P., C.G.M. [original signed]

   SUBJECT:  Insurance Companies:  United HealthCare, MAMSI, MDIPA, Optimum
                    Choice, Inc., and One-Net
 

Due to United HealthCare’s (known as “United”) inflexible refusal to our requests for improved reimbursement, I have been forced to not renew our contract with United.  United routinely and uniformly pays us anywhere from $2.86 to $29.16 for each patient’s visit; not including the patient’s co-pay.  The reimbursement I receive, as you can see, is so minimal and runs roughly $30-$50 less per visit from the other larger insurance providers.  I have remained with them as an obligation to my patients, but can no longer afford to do so.

 

I believe I will be out of network on December 14, 2009, but United has invented some road blocks and I may be in the plan through July 30, 2010.  Whether the earlier or the later date applies, I will be an out of network provider at some point.  I will post further details on the website and in the office.  For those of you who have Medicare as your primary, this does not affect you as Medicare sets what is to be paid for the services rendered. 

 

Thank you for entrusting me with your care.  I have considered it a privilege and do not want to lose you as a patient.  I know that open enrollment is happening for many of you right now, and I am hopeful that you will look into some of the other insurance options offered to you by your employer and change to an insurance where I am an in-network provider.

Please remember our new prescription policy that goes into effect 12/29/08 that has been posted in the waiting room since July 21, 2008 – you must make an appointment to get all prescriptions refilled; no exceptions. 


The number of refills will be determined as to when the Dr. will need to see you again.  Letter below if you have not been in our office since July 2008.

NOTICE TO OUR PATIENTS

To our patients:  Our practice has been growing rapidly and so your medical staff can provide you with even more one-on-one service, and to make sure you are continuing to take the proper medications for your condition(s), starting December 29, 2008, we will no longer fill prescriptions without an appointment.  We are also requesting that you bring all your prescription bottles in with you for your appointment to fill your prescription(s).  This will help us better monitor the medication you are taking and do any adjustments as needed.   Prescriptions will be filled on a three month or sixth month basis depending on the condition/illness (for example, our diabetic patients require a three month check-up, your prescription(s) will be filled for three month increments at the time of that appointment).
